Case management enriches social, behavioral, physical, and financial well being by growing an individual’s support network to address functional needs through collaboration. Through collaborative programming with municipalities and other community based organizations, we provide OST programming, group/family/individual therapy, case management, adult day care services, mentoring opportunities, and special events that improve social determinants of health for at risk populations and people with disabilities of all ages and backgrounds.
ASTRO is Currently seeking Registered Nurses who are looking to work as independent contractors in an adult day care facility. A nurse at ASTRO is responsible for caring for our members and helping them to achieve their personal health goals. This person will have excellent interpersonal skills and experience developing rapport and trust with clients along with administering care plans.
Job Responsibilities:
- Conduct assessments for clients
- Administer medications to participants and monitor these medications
- Performs skilled nursing care as ordered by the facility
- Observe and monitor patient’s condition
- Evaluates patient care and progress
- Documents all nursing care on the appropriate care notes
Qualifications:
- Required Education : Graduate from an accredited school of professional nursing
- Required license or certification:
- Current California RN license
- Current BLS/ACLS

Job Title: Mental Health Counselor

A counselor at ASTRO is primarily responsible for providing intensive home, school, or community-based Mental Health Rehabilitation services to children, adolescents, and adults. A counselor performs clinical assessments and conducts consultation through individual/group therapy for clients and/or their families. This position is part time and offers flexible work hours, according to the needs of our clients.
Qualifications:
Master’s Degree from an accredited college with a major in psychology, social work, counseling (with a clinical specialty in the social services field) or another related field. Licensure as a LICSW or LMHC is required.
Knowledge/Skills/Experiences:
- Knowledge of professional methods and techniques of mental health therapy and addictions counseling.
- Knowledge of various mental health, addictions and related illnesses methods of treatment.
- Knowledge of proper interviewing and therapy techniques.
- Knowledge of professional methods and techniques of individual, brief, group, family therapy.
- Ability to diagnose mental health, addictions, and related illnesses, develop and implement treatment plans under supervision.
- Recognizes and is sensitive to cultural and ethnic differences.
- Ability to communicate clearly both verbally and in writing (bilingual Spanish and English fluency is preferred)
- Ability to adapt to change in work or client assignment.
- Ability to demonstrate skill in crisis intervention situations.
- Presents integrated care case at monthly team meetings
- Consults with primary care and nurse to establish goals for the treatment plan related to health
Duties and Responsibilities may include but are not limited to:
- Performs professional mental health therapy and related duties in the treatment of clients having psychologically related problems.
- Provides intake and crisis services, develops treatment plans and participates in evaluation of treatment methods and procedures.
- Provides individual, brief, group psychotherapy and psychometric services to clients.
- Prepares and maintains necessary client records and reports in accordance with licensing and accreditation standards and agency policies/procedures.
- Documents client response to individual and group therapy in approved formats including individual and group progress notes.
- Participates in the Performance Improvement activities of the agency.
- Provides appropriate linkage and referral according to agency mission, values, policies, and procedures.
- Ensures that definition of individual rights and responsibilities of clients, their families, visitors, and co-workers are honored.
- Supports and implements the agency organizational and clinical policies and procedures.
- Participates in regular agency meetings, clinical supervision, and training as determined.
