At ASTRO we cater to the unique needs of the individual by recreating an approach to therapy that is based around recreational activities that interest and engage our clients. Our programs are designed to enhance Social Emotional Learning (SEL) in a therapeutic and supportive environment. By creating a setting that makes the individual feel comfortable and confident, we teach, model, and socially coach participants to help them find their own path towards emotional regulation, building and maintaining healthy relationships, and problem solving.​​

Information required for scheduling Intake Assessment
  • Referred client’s full name as it appears on medical insurance card

  • Medical Insurance ID number

  • Client’s DOB

  • Client’s address

  • Client’s diagnosis (if any, not required)

  • Service client is seeking (individual therapy, group therapy, family therapy or a combination of these services)

 

What to expect when setting up an Intake Assessment

  • All insurances must be verified and authorized (as needed) prior to being seen for an intake assessment.
  • Client’s are not allowed to be receiving a higher level of care such as EOS services at the time of intake. If you and your family are currently participating in EOS services, discharge from these services must be completed prior to completing an intake assessment with ASTRO.
  • No more than one service on an outpatient basis can be completed on the same day. This means if you receive outpatient services with other mental health care providers, you can not complete intake assessment (and/or any other services with Social Sparks) on the same day. Please be mindful of this when scheduling your intake assessment.
  • No more than 2 outpatient services are allowed per week per insurance.
  • Typical timeline for setting up an intake assessment is within a 2 week window from initial call. (during which time insurance verification and authorization will be completed).
  • At intake assessment, a copy of your health insurance card is required.
  • A parent and/or guardian is required to attend the intake assessment (we have a sibling friendly environment).
  • The referred client is highly suggested to attend the intake assessment.
  • Co-payment is due at time of intake assessment.
  • Evaluations, IEP’s and any other supporting documents are welcomed and appreciated at intake assessment. (We are able to make copies as needed)

Attendance and Copay Policy

Please note that ASTRO has a 24 hour cancellation policy for all services including individual, family and group therapy. If you are unable to keep your scheduled appointment, we ask that you call the office 24 hours in advance to notify the front desk of you or your child’s absence. Clients will be charged a $25 fee if they are a “no show, no call” to their scheduled appointment.
